Professional Discretion
FMLA and ESA documentation services are provided at my professional discretion only. Please be aware that I do not guarantee these services to every client, as each case must undergo a thorough individual clinical review to ensure it meets appropriate standards of care.
Session Requirements
To ensure I have sufficient clinical information to support your request ethically and accurately, I require a minimum of three consistent therapy sessions before I will consider completing any FMLA or ESA paperwork. This period allows for a meaningful clinical assessment of your needs.
Paperwork Fees
The completion of FMLA and ESA paperwork requires additional time for clinical review and administrative preparation. Therefore, there is a separate fee for these services, which is billed in addition to your regular therapy session fees.
Clinical Assessment Standard
Paperwork completion is based strictly on a clinical assessment and professional treatment needs, rather than solely upon a client's request. If it is determined that the documentation is not clinically appropriate or reflected in the treatment plan, I reserve the right to decline the request.
