NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
THE WAY THROUGH THERAPY SERVICE is dedicated to maintaining the privacy of your protected health information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you. We also are required by law to provide you with this notice of our legal duties and the privacy practices that we maintain in our practice concerning your PHI.
1. HOW WE MAY USE AND DISCLOSE YOUR PHI
Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and any related services. For example, your provider may share your PHI with another consultant involved in your care.
Payment: Your PHI may be used to obtain payment for your healthcare services. This may include certain activities that your health insurance plan may undertake before it approves or pays for the healthcare services we recommend for you.
Healthcare Operations: We may use or disclose your PHI in order to support the business activities of THE WAY THROUGH THERAPY SERVICE or your provider.
2. YOUR RIGHTS REGARDING YOUR PHI
Right to Inspect and Copy: You have the right to inspect and copy health information that may be used to make decisions about your care. Usually, this includes medical and billing records.
Right to Amend: If you feel that health information we have about you is incorrect or incomplete, you may ask us to amend the information.
Right to an Accounting of Disclosures: You have the right to request an “accounting of disclosures.” This is a list of the disclosures we made of health information about you for purposes other than treatment, payment, and healthcare operations.
3. COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with THE WAY THROUGH THERAPY SERVICE or with the Secretary of the Department of Health and Human Services. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
THE WAY THROUGH THERAPY SERVICE reserves the right to change the terms of this notice and will inform you by posting a notice of any such change on our web site.
